How to complete the CIS Debit Order Authorization Form

This form should be completed in consultation with support @ cis.za.net or a CIS representative. The purpose of the form is to provide CIS with the necessary authority to debit the customer's bank account for the cost of services rendered each month. If you have elected to pay annually you do not need to complete this form.
